Welcome to Collaborative Counseling’s Patient Portal!
In order to better serve you, Collaborative Counseling offers you the ability to complete forms and communicate with us through our Patient Portal. We offer this service through a certified interoperable electronic health record that is in compliance with Minnesota and Wisconsin state law.
If you choose to utilize the Patient Portal, this secure account will allow you to:
• Update your contact and billing information
• Complete and submit paperwork online before appointments
• Email your provider(s)
All clients are offered a patient portal when you call to schedule as a new client. You can also request we set one up for you by asking your therapist at your appointment or by calling our administrative support at 763-210-9966.
IMPORTANT: If you complete documents or measures in our portal, they will not be reviewed by your therapist until your appointment. If you are struggling with any safety concerns in which you are concerned about safety for yourself or someone else please call the Crisis Connection at (612) 379-6363 or call 911. When you login to your portal you will see the following tabs outlined below. After completing the initial account setup, take a few moments to become familiar with the tabs in the client portal:
This is where you can find intake forms to complete online prior to intake. The Patient Portal also gives your provider(s) the ability to send you “clinical measures” before your appointments. These measures are questionnaires and rating scales that are used to help measure symptoms or collect clinically significant information the impact your care. If these have been assigned, please take time to click on and complete each form listed prior to the due date. This takes just a few minutes and all answers are kept confidential and read only by your provider.
When new measures are assigned, you will receive an email letting you know that there are measures in your portal that we would like you to complete prior to your next appointment.
Currently we are not using this tab in the Patient Portal but hope to in the future!
You will notice that there is a Quick Link on the Home Tab for “Change demographic or insurance information”. Review your current information for accuracy, if you make any changes, just submit and these changes will automatically populate the demographic fields in your patient account at Collaborative Counseling. These changes can also be made in the Account Settings tab as well.
If you are a new client, you can complete the demographic and insurance information and submit. This will automatically populate the demographic fields in your patient account with us.
Under this tab, you will find our Release of Information form and the Collaborative Counseling’s full up to date policies. You can choose to print these forms and bring them with you if you would like and are able. If you are unable to print these before you come we will also have these forms available in the office for your first appointment.
This tab will show you all of our locations and addresses to help you easily navigate to your appointment with us.
From the Messages tab, you can send your provider(s) secure emails. These messages are secure HIPPA compliant messages. They utilize the messaging system in Valant and are not a part of a regular email system.
